动火工作票制度
为贯彻电力工业“安全第一、预防为主”及消防工作“预防为主、防消结合”的方针，加强动火管理工作，保障设备和人身安全，依据《电力设备典型消防规程》，特制订本制度。
本制度适应于全厂防火重点部位或场所以及禁止明火区的动火管理工作。
一、动火工作票的填用
主要部位或场所有：
(1)配电室、高压试验室、化验室
(2) 油库及油库围墙以内;
(3) 电缆沟及电缆夹层间;
(4) 油处理区域范围之内
(5)化工仓库
二、动火工作管理规定
l、动火工作票应事先编号，未经编号的动火工作票不准使用。
2、动火工作票应用黑色或兰色笔填写，不准使用红色笔或铅笔。填写要清
晰，不得任意涂改。
3、动火工作票至少一式三份，一份由工作负责人收执，一份由动火执行人
收执。动火工作终结后应将这两份工作票交还给动火工作票签发人。一级动火
工作票应有一份保存在厂安监部，二级动火工作票应有一份保存在动火部门。
若动火工作与运行有关时，还应多一份交运行人员收执。
4、动火工作票签发人不得兼任该项工作的工作负责人。动火工作负责人
可以填写动火工作票。
5、动火工作票的审批人、消防监护人不得签发动火工作票。
6、动火工作票不得代替设备停复役手续或检修工作票。
7、动火工作间断或终结时应清理现场，认真检查和消除残留火种。
8、已执行的动火工作票均应在票的右上角加盖“已执行”章。每月由车间
统一收回保存三个月。
9、各级人员在发现防火安全措施不完善不正确时。或在动火工作过程中，
发现有危险或违反有关规定时。均有权立即停止动火工作。并报告上级防火责
任人。
10、外单位来生产区内动火时，应由负责该项工作的本厂人员按动火等
级履行动火工作票制度。
11、遇下列情况之一时，严禁动火：
(1)油车停靠的区域;
(2)压力容器或管道未泄压前;
(3)存放(或残留有)易燃易爆物品的容器(或管道)未清理干净前
(4)风力达5级以上的露天作业;
(5)遇有火险异常情况未查明原因和消除前。
三、动火工作票中所列人员的安全责任
1、各级审批人员及工作票签发人的安全责任
各级审批人员及工作票签发人应审查;
(1)工作必要性;
(2)工作是否安全;
(3)工作票上所填安全措施是否正确完备;
2、运行许可人的安全责任
运行许可人应审查;
(1)工作票所列安全措施是否正确完备，是否符合现场条件;
(2)动火设备与运行设备是否已隔绝;
(3)向工作负责人交待运行所做的安全措施是否完善;
3、工作负责人的安全责任
工作负责人应负责;
(1) 正确安全地组织动火工作;
(2)检修应做的安全措施并使其完善;
(3)向有关人员布置动火工作，交待防火安全措施和进行安全教育;
(4)始终监督现场动火工作;
(5)办理动火工作票开工和终结;
(6)动火工作间断、终结时检查现场无残留火种;
4、消防监护人的安全责任
消防监护人应负责
(1)动火现场配备必要的、足够的消防设施;
(2)检查现场消防安全措施的完善和正确;
(3)监督动火部位或现场可燃气体和可燃液体的可燃蒸气含量或粉尘浓
度符合安全要求;
(4)始终监视现场动火作业的动态。发现失火及时扑救;
(5)动火工作间断、终结时检查现场无残留火种;
5、动火执行人的安全责任
(1)动火前必须收到经审核批准且允许动火的动火工作票;
(2)按本工种规定的防火安全要求做好安全措施;
(3)全面了解动火工作任务和要求，并在规定的范围内执行动火;
(4)动火工作间断、终结时清理并检查现场无残留火种。
四、动火审批权限
动火工作票由申请动火部门负责人或技术负责人签发
负责人审核，生产副总批准。
五、动火工作票的执行程序
1、签发工作票
(1)工作票签发人根据工作任务的需要和计划工作期限确定工作负责人。
(2)工作票一般应由工作票签发人填写一式二份。签发时应将工作票全部容向
工作负责人交待清楚。工作票由工作负责人填写时填写后交工作票签发人审核。
工作票签发人对工作票的全部内容确认无误后签发，并仍应将工作票全部内容向工
作负责人作详细交待。工作票应由工作负责人送交运行班长。
2、接收工作票
(1) 工作票应在开工前一小时，送交运行班长，由运行班长对工作票全部内容进
行审查;必要时在“运行应采取的安全措施”，栏内填好补充安全措施，确认无问题
后记上收到工作票时间，并在运行班长处签名。
(2) 审查发现问题应向工作负责人询问清楚，如安全措施有错误或重要遗漏，工
作票签发人应重新签发工作票。
(3)运行班长签收工作票后，该工作票应送交值长(单元长)审批。
3、安全措施的审批、执行及工作许可
(1) 由检查人员对动火现场测定可燃气体、易燃液体的可燃蒸气含量或粉尘浓度，
并签注意见。
(2) 检查人员签注上述气体含量或粉尘浓度合格、可以进行动火作业的意见后，
由各级审批人员对工作票进行审核批准。
(3)根据工作票布置执行安全措施
(3) 安全措施全部执行完毕.并经消防监护人认可，工作负责人、运行许可人、
动火执行人共同到现场检查，无误后办理开工手续。
4、开始工作
(1) 动火工作开始前，工作负责人应将分工情况、安全措施布置情况及安全注意
事项向全体工作人员交待清楚后，方可下达开工命令。
。
(2) 工作负责人和运行许可人不允在许可开工后单方面变动安全措施。如需变动时，
应停止工作并经双方同意，重新履行动火工作票制度。
5、动火现场的监护
(1)动火在首次动火时，各级审批人员和动火工作票签发人均应到现场检查
防火安全措施是否正确完备，测定要可燃气体、易燃液体的可燃蒸气含量或粉尘浓度是
否合格，并在监护下作明为试验，确地无问题后可动火作业。
(2)动火时，动火部门负责人或技术负责人应始终在现场监护。
(4)动火工作票在次日动火前必须重新检查防火安全措施并测定
可燃气体、易燃液体的可燃蒸气含量或粉尘浓度，合格方可重新动火。
6、工作终结
(1)动火工作完工后，工作负责人应全面检查并清理现场确认无残留火
种后带领工作人员撤离现场。
(2)工作负责人、动火执行人会同运行许可人、消防监护人共同到现场检
查验收。确认无问题时，办理终结手续。
六、附则
本制度如与上级有关规定制度有抵触时，以上级有关规定制度为准。
